Intake Form for Existing Clinic Clients

The information requested below is to help your pet’s health care provider to learn more about your pet before the appointment. This will let us spend more time addressing any concerns that you may have while you are in the office. Please fill this form out as completely as possible.

Please note that the form on this page is only for existing Clinic clients. An “existing Clinic client” would be a person who has brought any pet in to the BloomingPaws Vet Clinic that was seen by our veterinarian within the last two years. If this is the case for you, you will be considered an “existing client” even if you are now going to bring in another pet for the first time. Filling out this form will save you some time.

If you are not an existing Clinic client, please answer “No” to the question below, and you will see a text box with a link that will take you directly to our New Client form on another page. Resort clients who have not been to the Clinic with a pet within the last two years should also answer “No” to the question below.
